The Practice will be installing a new Electronic Patient Record System called SystmOne on the 20th June 2019.The new system will bring substantial benefits for both patients and staff.
Patients currently registered for 'Patient Access' will need to re-register with SystmOne Online. Forms will be available from reception after 1st July. Patient Access will no longer be available from 7th June until SystmOne Online goes live in July and you have re-registered. Therefore, you will not able to book appointments or request prescriptions online at this time, Please book your appointments at the surgery or over the phone. Repeat prescriptions can be ordered in writing or by using your repeat slip. We thank you for your patience during this time of transition. Penny’s Hill Practice is a six partner practice providing a range of primary care and community health services from Ferndown Medical Centre. The practice has been a training practice since 1979 with regular placements of GP Registrars. The practice shares the purpose built premises with Orchid House Surgery with a range of community services also being provided from attached facilities. These include school health, district nursing, health visiting, speech and language therapy and chiropody.
Our practice boundary is bounded by the River Stour to the South, the Stapehill Roundabout to the West, West Moors Army Depot to the North and Azalea roundabout to the East. This has recently been extended in the north along the A31 to include St Leonards, St Ives and along to Three Legged Cross, and in the West to include Broom Hill, Colehill and back along Wimborne Road West to the Canford Bottom Roundabout. Please note the extended area has been agreed to accomodate any of our existing patients currently living within the inner boundary, who choose to make a local move to within the marked outer boundary. Please bring your medical card to the surgery and a receptionist will register you with a doctor. If you do not have a medical card, please ask at reception for an application form. Please also see the Pre-registration tab.
Temporary residents are registered on a special form at reception.
This practice has a system of personal lists. This means that although you sign on with the practice, you will be registered with a named doctor and will be seen by that doctor. We believe this gives greater continuity of care for our patients. Where possible the practice will endeavour to ensure that we respect your right to express a preference of practitioner. However, this may not always be possible and if so, an explanation will be offered.
New patients will be given a health and medical questionnaire to complete. This is important as it allows us to get to know you better and collect important information on past medical health, current problems and medication. It is also a good time to ensure that all immunisations, contraceptive advice, cervical smears and blood pressure measurements are up to date. The new patient information is vital to the doctor. Once registered your records will automatically be requested from your previous Practice, this may take several weeks. If your previous Practice has the capacity to send the records electronically, this can happen within a week.

Sending this form will NOT automatically register you with this pratice. Your details will be held at the surgery for a limited period of time until you are able to present in person to sign your registration form to confirm the details are correct and to also provide hard copy proof of your ID and address.
Pre-registration Form When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
